Support coordination for ndis participants

Support Coordination is designed to help NDIS participants understand their plan and connect with the right services so they can achieve their goals.

At Only the Best Support Coordination 4U (OBSC4U), Support Coordinators work alongside participants to help them navigate the NDIS, identify suitable providers, and ensure supports are working effectively.

Our focus is on helping participants build the confidence and skills to manage their supports while ensuring the right services are in place.

Our Support Coordinators assist participants to:

• Understand their NDIS plan and available funding

• Identify supports that align with their goals

• Connect with suitable service providers

• Coordinate supports and appointments

• Resolve issues when services are not working well

• Monitor progress and make adjustments when needed

• Prepare for plan reviews

Support Coordination is always provided in a way that promotes participant choice and control, ensuring participants are able to make informed decisions about their supports.

An important part of Support Coordination is helping participants build a reliable team of providers. OBSC4U works collaboratively with participants, families, allied health professionals, support workers, and other providers to ensure services are coordinated and working together effectively.

OBSC4U supports participants across Newcastle, Lake Macquarie, Maitland, Port Stephens and Cessnock, with phone and online support available outside these areas.
